Thời VNCH hiểu sai và coi thường nursing
VN hien nay thì "không có chó bắt mèo ăn cứt " tổ chức Y sĩ là một kiểu nurse practitioners cuả Mỹ , nhưng huấn luyện quá thấp
Có lẽ vì các Bác sĩ lãnh đạo Y tế cả hai mien thời đó trình độ không đủ cao để thấy cái quan trọng cuả nursing
Khuynh hướng cuả tổ chúc y tế Mỹ trong tương lai gần là tổ chức những walk-in clinics, giao cho Nurse Practitioners khám.
Y khoa bây giờ quá cao và y khoa phòng ngưà quà hiệu quả .
Một BS học 8 năm với it nhất 3 năm training làm out patient clinics chỉ làm chức năng cuả nurses, tiền trả cho BS cao hơn nurse practitioners .Nhiêu supermarkets đã tổ chức pharmacies + clinics.

Nói thêm cho YFs biết về Nursing ở Mẽo. Trước hết phải biết rằng trong cái Anglo-Saxon culture cuả người da trắng gốm các nước Anh, Ái Nhĩ Lan, Tô Cách Lan, Úc, Gia Nã Đại, Tân Tây Lan và Huê Kỳ thì ngành Nursing có truyền thống cuả Florence Nightingale nên rất tận tâm và có professionalism. VN chỉ biết Tàu và Tây nên không biết cái truyền thống cao quý đó cuả Nursing.
Nurse ở Mẽo có nhiều loại tuỳ theo trình độ học vấn. Thấp nhất là Nurse Aid và cao nhất là Nurse Ph.D.
1-Nurse Aid: high school level với 1 certificate of training (thường là 6 tháng), có thể tắm rưả bệnh, giúp di chuyển bệnh, đút đồ ăn cho bệnh nhân etc….
2-License Practical (Vocational) Nurse : 2 year post high school, LPN license làm như nurse aid và có thể đo vital signs, ghi hồ sơ, phát thuốc theo toa, chích kim etc…
3-Register Nurse with Associated Degree: Làm như LPN, supervise nurse aid và LPN, ghi hồ sơ, gọi BS khi cần thiết để tuờng trình vấn đề, ghi order cuả BS bằng telephone lên hồ sơ.
4-Register Nurse with Bachelor (BSN) Degree: Đây là College Educated Nurse, làm như Register Nurse, làm Charge Nurse (Sếp Ca), có thể học thêm để làm Nurse Practitioner, Nurse Specialist.
5-Advance Practice Nurse (APN) phải có Master Degree : có thể xem bệnh, chẩn bệnh cho thuốc khi có sự hợp tác cuả BS (Physician), có thể chuyên môn ví dụ Nurse Anesthetist, Nurse Specialist etc… Lúc tui còn là Vice Chairman cuả Department và Associate Chief of Staff cho Geriatrics and Extended Care cuả bệnh viện, tui mướn rất nhiều APNs cho Nursing Homes, Rehab Facilities và Home Care Services.
6-Nurse Ph.D. làm research, dạy College of Nursing etc…
Ngoài ra còn có Nurse with BSN đi học Master of Public Health để làm Public Health, Phòng Ngưà, Dịch Học, Education etc…
Có tụi không phải Nurse, học làm Physician Assistant (PA) Master Degree level làm phụ tá cho BS trong quân đội, trại mổ etc….
Tui làm việc với Nurse Ph.D. và có co-authored vài publications với họ hồi đó.

The moot point is the business side of this tendency.The training of a nurse practitioner is around 6 years.The training of a PCP is 11 years.To satisfy the requirement of more primary care practitioners in this country, it is more economical to produce more nurse practitioners or PA.The public does not know and the pay is the same in primary offices.We see now some clinics and even hospitals staffed by physician extenders.Some doctors hire these extenders and can double or triple the income.I do not want to broach on liability issues.It is unfair competition.There is nothing we can do.

APN and PA are not "Licensed Independent Practitioner" , they need a supervising or collaborating physician (MD or DO) in their "scope of practice". I hired them at first for the Nursing Homes; later, when it was obvious that their presence at NH improved the medical care of NH patients without increasing the expenses, I put them in rehab facilities and primary care clinics as well. My MD faculty members love to have them because Interns and Residents come and go with rotation but the PAs and APNs will always be there to help. Dr. TĐ should go with the flow.
